According to numerous seminal research by the International Atomic Power Agency (IAEA) and other sources (to be cited later), the root cause of the Chernobyl nuclear energy plant accident was attributed to a mainly deficient safety culture, not only at the Chernobyl plant but also all through the Soviet design and style, operating and regulatory oversight for nuclear energy that existed at the time of accident in 1986.
